When a child has medical treatment, the self-borne portion of the expense on health insurance program will be returned to you as subsidy by the city. In order to apply for such subsidy the new card will be necessary to be shown. So, please apply for the card to begin with. The application is acceptable by mail.
Eligibility
Children of ages zero up to third graders of middle school (15 years old) living in Bunkyo city are eligible.
There are two different cards:
(1) Infant Medical Card for pre-school children.
(2) Child Medical Card for school children(up to 15 years old)
*No income ceiling is set for the applying family.
*The families on the welfare program and the children in social welfare institutions are excluded.
Subsidy Details
Self-borne portion of medical expenses which are to be covered by public health insurance program for the children’s hospital visit and/or hospitalization except for
(1) Meals
(2) Expenses which are not to be covered by medical insurance program such as medicine container and/or the rental fee, regular health checkup, document fee,extra-room such as private room or rooms for fewer patients.
(3) Treatment for the injury took place at schools, kindergartens and nursery schools, which is to be paid by National Agency for the Advancement of Sports and Health.
